If you participated in the Healthcare or Dependent Care Flexible Spending Accounts (FSAs) in 2009, any remaining claims for last year must be postmarked no later than March 31, 2010.

Any unclaimed money remaining in your 2009 FSA accounts after March 31, 2010 will be forfeited.

To submit a claim, please use the FSA Reimbursement Request Form (PDF).

Please fill out, print and mail the form, along with any required receipts, to the address listed on the form.

TRANSIT/PARKING REIMBURSEMENT PROGRAM

Unlike Flexible Spending Accounts, unused money in a Transit/Parking Reimbursement Program (T/PRP) account will roll over into your 2010 account.

The rollover money will be available in your account after June 1, 2010. In the meantime, please download a T/PRP Reimbursement Request Form (PDF) to submit a claim.
